Olive Springs Retreat invites you to nominate someone or submit your own story for the chance to win a beauty makeover and training on how to keep it up!

2. The story behind the look
3. Why nominee (or you) needs a new look
4. Contact information; including phone, address, email

The final winner will be chosen by Katie Lott. All services will be provided by Katie Lott. The new look may include any or all of the following:

1. Hair color
2. Hair cut
3. Brow shaping
4. Make-up application
5. Clothing discounts with Modbe
6. Any additional services needed to complete the new look

Requirements
1. Must be open to BIG changes
2. Must be new to Olive Springs Retreat
3. Must live within 40 miles of 84043
